Apartment Preference

The following information will be used in assigning the apartment.  All of the preferences and comments provided below will be considered; however, apartment selection is solely based on availability at the sole discretion of the Landlord.

Pets

We have a 1 pet maximum and a weight limit of 50 lbs. Pets are limited to the first and second floor units only. Additionally, dogs of the following breeds are not permitted (purebred or mixed breed): Pit Bull (a.k.a. Staffordshire Terrier or Bull Terrier); German Shepherd; Husky; Malamute; Doberman Pinscher; Rottweiler; Chow Chow. Pets must be registered with the leasing office along with a pet photo and veterinary records. A pet fee and monthly pet rent will be assessed; call the leasing office for more information.



Terms and Conditions

Applicant represents all of the above information is true, complete and authorizes the verification of same by reasonable means.  Any misrepresentation or omission may be cause for the rejection of this application and/or termination of this lease.  Applicant further authorizes free exchange of information between Western University of Health Sciences and Capstone Management, including but not limited to the applicant's student status, academic records, judicial records, and financial records to be used to determine whether this applicant shall be approved or disapproved.

Applicant understands the application fee of 45.00 (per Tenant) is non-refundable and non-transferable.  This fee is used to process credit history of applicant and/or Guarantor.
